Kaiser Health News recently partnered on two articles about insurance coverage.  The first explores 2015 plans lacking hospital coverage due to a faulty calculator designed by Health and Hospital Services (HHS).  Until fixed, this could negatively impact low-wage employees. The second focuses on pay limits on procedures, such as knee replacements, and use of controversial reference pricing. The Affordable Care Act (ACA) allows large group and self-insured plans to use reference pricing. If you choose out-of-network providers for such services, the amount won't count toward out-of-pocket maximums.
